The following are the forms needed for program registration. Simply look at the program you want and then download the appropriate forms from the list below.

 

To begin the application and enrollment process for Either Afterschool Program you MUST contact the program director first. You may contact the program director by calling (707)-826-1915 during normal operating school hours. You may also contact the program director by email directly at [email protected].




After School Program

 

     Current Students registering for the 2024-25 school year only need the schedule form below.

 

    New Registration materials needed (After being approved by the program director due to space )

If your family is currently enrolled in one of the programs and wants to add the other program, all we need is a new contract and Schedule for the appropriate program after contacting the program director first for approval.
